Dark pool share: 34% off-exchange — 17pp below the market
Off-exchange prints never touch the public book, so they move size without moving the quote. That is the point of using them. Most of the day cleared on the lit market — either there was nothing to hide, or the large players sat out. Short share of the off-exchange piece: 56% vs a 63% norm.

What it means: Options positioning leans defensive—put-heavy hedging (0.68 put-to-call ratio) with minimal squeeze pressure—while dark-pool activity is subdued and below normal (0.81x typical volume), suggesting institutional caution despite the stock trading near max pain ($400 vs $417.82 price). No fresh large options positions opened yesterday to confirm conviction either way.
News vs the money
⚡ DIVERGENCECan HCA's cost controls protect profits as insurance companies push back?
The story frames operational strength as a profit shield, but options traders are holding more downside insurance (puts) than upside bets, and off-exchange volume is running below normal—a mismatch between optimistic narrative and cautious positioning.
Zacks Investment Research
⚡ DIVERGENCEAnalyst values HCA at $785 per share, but stock trades at $429—is it a bargain?
The DCF case is bullish on valuation, yet options traders are net-short volatility with put-heavy positioning and near-zero squeeze risk, indicating the market is not pricing in a reversion to $785.

Bank of Nova Scotia buys $18.5 million stake in HCA Healthcare
While the 13F filing shows institutional entry, options hedging remains elevated and dark-pool accumulation is below normal, suggesting this purchase is routine portfolio allocation rather than a conviction bet.

B. Metzler seel. Sohn & Co. AG acquires 10,563 shares of HCA
Small institutional position additions are consistent with neutral positioning; no options flow or dark-pool surge backs this as a meaningful conviction move.

Advisors Capital Management LLC builds 6,507-share position in HCA
Routine institutional accumulation in small tranches is consistent with the neutral dark-pool regime and low squeeze pressure; no conviction signal from options or off-exchange flow.

What is a “divergence”?
A divergence is when the news narrative and the institutional money flow point in opposite directions — a bearish headline while large call premium is bought, or heavy dark-pool selling under a bullish story. It signals the crowd and the desks may disagree.
How to read these numbers
Dark-pool volume — The share of trading executed off-exchange, at wholesalers and dark pools. About half of all US share volume prints there on an ordinary day, so the level by itself says almost nothing. What carries information is the distance from this name’s own recent norm, and whether the off-exchange size ran above its usual.
Off-exchange short share — How much of that off-exchange volume was sold short. The median across all listed names is about 49%, because wholesalers filling retail buy orders sell short and cover later. A reading near half is plumbing, not a bearish vote — compare it to the same name’s own 20-day norm.
Max pain — The price where the most options expire worthless — positioning often gravitates toward it near expiry.
Call wall / Put floor — Strikes with the heaviest call/put open interest — they often act as short-term resistance and support.
Put/Call ratio — Below ~0.7 leans bullish (more calls); above ~1 leans defensive (more puts).
